Includes conversation starter kits, information for health care proxies, talking to your health care team, and identifying what matters to you. Special guides available about Alzheimer's and other dementias as well as caring for a child with serious illness.

Tips and Suggestions: Introduce learners to these resources by having them complete the guide as if they are a patient (or as themselves) and role play conversations with another student. Then debrief how they felt. This will help them gain perspective as to how patients may feel thinking about and talking about these topics with healthcare professionals.
