Have you met the POLST form yet?

Meet & Greet with the POLST Form on Wednesday, July 28, 2021, 12:00-1:30pm ET

In March 2021, Massachusetts officially began the important work of transitioning the current paper-based MOLST Program to the much improved POLST electronic form and statewide electronic registry. These changes, which we’re calling the MOLST-to-POLST process, hold the promise to make our program an even more powerful, inclusive, transparent and meaningful tool for caring for the seriously ill.

The Coalition is partnering with the state to support this effort and has formed a MOLST to POLST Advisory Group that brings together healthcare and community leaders to synthesize their collective knowledge and share their diverse stakeholder experiences.

We want to hear your ideas on how to make this a successful transition and will be holding virtual sessions for our broader community to come together, learn about the process, and provide input.

From the Betsy Lehman Center for Patient Safety, a Coalition member: Health care facilities in Massachusetts are beginning to ease a series of unprecedented visitation restrictions in place since early 2020. They are now taking stock of lessons learned over the past 15 months about coronavirus transmission, infection risks and the consequences of sequestering patients and people who live in residential care settings away from family caregivers and friends.
